Two primary ver sions of questionnaires were used: a short form containing on!y the 100-percent questions and a long form containing both the 100-percent questions and the additional sample questions. Appendix E presents facsimiles of the question naire pages and the respondent instructions used to collect the data included in this report. Appendix F lists the subjects that are covered by the 100-percent and sample components of the 1990 census.
