Tracy J. Mayne, PhD, is Director and Global Renal Anemia Area Lead in Global Health Economics at Amgen, Inc, California, USA. John C. Norcross, PhD, ABPP, is Professor of Psychology and Distinguished University Fellow at the University of Scranton, USA, and coauthor of Leaving It at the Office and Authoritative Guide to Self-Help Resources in Mental Health.
