WAYNE KATON, MD, was Professor, Director of the Division of Health Services and Epidemiology, and Vice Chair in the Department of Psychiatry and Behavioral Sciences at the University of Washington. KARI A. STEPHENS, PHD, is a clinical psychologist and Assistant Professor in Psychiatry and Behavioral Sciences at the University of Washington.
